CHANGE OF ADDRESS

Please keep us informed of any change in your address or phone numbers. 

Per banking regulations, a customer's signature is required to change the mailing address.  Please download our Change of Address form and complete all the applicable information including the name of other family members effected by the address change.  Please update all phone numbers and e-mail addresses.  Sign the form and return it to us in person, by mail or fax to 765-436-2522.
